The Alice Cooper and the Tome of Madness slot features a 5x5 Cluster Pays grid. Winning combinations are formed by landing groups of symbols. These clusters can be vertical, horizontal, or a combination of the two. Winning clusters are then removed from the screen using the cascade mechanic, which fills the holes with new symbols and stops when no new clusters develop. 

Lower-value symbols include four skull icons, while higher-paying symbols include spiders, crosses, snakes, and hearts. The most lucrative clusters have 15 or more matching symbols, and premium clusters of this size are worth between 40 and 200 times the stake. Another sign for this is the standard Tome wild, which replaces all normal pay symbols. On non-winning spins, one of the Reality Effects modifiers may be randomly activated. Insane Asylum combines all high payments into a single high pay symbol. The Dungeon changes all low-wage jobs into various high-wage jobs. When keys are collected, they leave behind a wild symbol. When three are collected in a single spin, they activate the stacked Alice Cooper Mega Wild when there are no more wins. The Mega Wild appears on reels 2, 3, or 4, and moves left or right between cascades until disappearing completely.

If Reality Spins are completed, just the highest tier is awarded. The game then transitions to a selection menu where players can select Reality Spins with either the Insane Asylum, The Dungeon, Chamber of Horrors, or Temptation Room Reality Effects. Whichever Reality Effect is selected will play at the end of each Reality Spin. If the Alice Cooper Mega Wild is introduced to the reels during Reality Spins, it has the potential to add 3 to 6 more wilds to the reels as it walks.
